Job Summary:
Provides professional nursing care through skillful assessment, diagnosis, outcomes identification, planning, implementation, and evaluation in accordance with facility or department policies procedures. Directs and manages the care of designated patients. Utilizes effective and appropriate communication styles. Assists in maintaining a safe work environment and maintains clinical and professional competency as appropriate to the population of patients served.Education – Required:
  • Specialized Diploma Nursing

Education – Preferred:

  • Bachelor’s Degree Nursing

Experience – Required:

  • 1 year Recent direct care experience in a healthcare setting.

Experience – Preferred:

  • 1 year Home Health experience.

Knowledge, Skills, and Abilities – Required:

  • Ability to build comprehensive reports and effectively convey information to others.
  • Ability to deal with crisis situations and make independent judgments.
  • Knowledge of patient education techniques and principles.
  • Knowledge of home care Conditions of Participation.
  • Skill in providing patient care, ensuring safety, and quality of nursing care.
  • Basic computer skills and ability to perform duties with accuracy and attention to detail.
  • Demonstrates flexibility and adaptability in varying situations to meet patient care needs.
  • Demonstrates initiative and ability to function independently without direct supervision and as a member of the care team.

Knowledge, Skills, and Abilities – Preferred:

Licensure – Required:

  • Registered Nurse PA RN License Upon Hire and
  • Drivers License valid PA driver’s license Upon Hire
  • CPR – Cardiopulmonary Resuscitation within 30 Days and
  • BCLS – Basic Life Support within 30 Days

Licensure – Preferred:

  • CPR – Cardiopulmonary Resuscitation Upon Hire and
  • BCLS – Basic Life Support Upon Hire

Additional Qualifications:

  • Home Health – proof of car insurance at PA State minimum requirements.Home Health – access to a reliable automobile.
Influenza vaccination is required at most LVHN facilities for all colleagues with patient contact unless medically contraindicated or due to a bona fide religious belief or practice, and is strongly encouraged for all colleagues.
