SPEECH LANGUAGE PATHOLOGIST – HOME CARE PER DIEM
JOB SUMMARY:Â
Evaluates and treats patients in the home in need of speech, language, cognitive, communication, voice disorders and dysphagia rehabilitation services based on agency policies and procedures. Establishes and implements treatment programs in accordance with physician orders. Communicates and coordinates care with other members of the interdisciplinary team. Reports to Supervisor of Therapy Services.
REQUIREMENTS:Â
Master’s Degree in Speech Language Pathology from an accredited/approved school as recognized by the ASHA. Minimum of 1 year experience as a Speech/Language Pathologist in an acute care or rehab setting required. Home Health Experience preferred.
Must be flexible to meet patient care needs and travel to patient care assignments over a multi-county area. Basic computer skills required. Ability to perform duties with accuracy and attention to detail. Ability to work weekends and holidays as needed to meet patient care needs.
LICENSURE/CERTIFICATION REQUIREMENTS: Current PA licensure Speech Language Pathology. Must have a valid driver’s license and access to reliable transportation. Must have current auto insurance at PA State minimum required levels. Current BLS Health Care Provider Certification.
